Position: Aftercare Social Care Worker

Location:    Family Hub, Drumcondra

Work Schedule: Full Time 38.5 Hours per week Monday to Friday

Contract:  Full Time Permanent

Salary:  € 39,625 - €49,205 Depending on Experience

Reporting to:  Manager / Deputy Manager

The Role: Social Care Worker


General Responsibilities:

  • The Aftercare Worker will provide structured, person-centred support to families transitioning from homelessness into long-term accommodation. The role aims to promote tenancy sustainment, enhance independent living skills, and improve overall family stability and wellbeing through coordinated, strengths-based interventions.

  • Establish and maintain professional, trusting relationships with service users and families, ensuring all interactions are conducted with dignity, respect, and in line with the values of Crosscare

  • Provide comprehensive aftercare support to families transitioning from emergency accommodation into long-term housing

  • Assist families in settling into their new homes, including support with utilities, budgeting, and household management

  • Deliver guidance on tenancy obligations and promote effective tenancy sustainment practices

  • Develop, implement, and review individualised, person-centred support plans tailored to each family’s needs

  • Maintain accurate, timely, and confidential case records in accordance with organisational systems (including PASS) and internal procedures

  • Support  access to essential services, including healthcare, education, childcare, community resources, and employment supports

  • Advocate on behalf of families with landlords, local authorities, and relevant service providers to ensure appropriate supports are secured

  • Conduct regular home visits to monitor progress, assess needs, and provide ongoing support

  • Identify potential risks to tenancy sustainment (e.g., rent arrears, neighbour disputes) and implement early intervention strategies

  • Ensure full compliance with safeguarding and child protection policies; respond appropriately to any concerns and complete all required follow-up actions

  • Adhere to  Crosscare policies and procedures

  • Work collaboratively as part of a multidisciplinary team, maintaining flexibility and responsiveness to reasonable requests from the Project Leader/Deputy

  • Prepare and submit written reports on service delivery and client progress as required

  • Comply with all Health and Safety policies and actively contribute to continuous quality improvement initiatives

  • Participate in supervision, team meetings, and maintain effective communication within the team

  • To provide cover to Crosscare Family hub team as required

  • Attend and contribute to internal and external meetings, training sessions, conferences, and other relevant professional development activities


Qualification Required & Experience Required

Education Requirements: 

  • Minimum level 6 on the National Framework of Qualification in Social Care Addiction, Mental Health, Nursing, Psychology

  • Full Driving License  is Essential

  • A minimum of 1 year relevant employment working with similar client group

  • Child Protection Training

 Experience

  •  External candidates- A minimum of 1 year working in the area of homelessness & housing

  • Awareness of the issues arising for residents/families moving into their own independent tenancy.

  • Experience of managing complex and challenging situation in a client based service.

  • Knowledge of Children’s First & Child Protection

  • Good time Management and organisation skills

  • Confident in making decisions and prioritising tasks while managing a caseload

  • Able to assess risk and take appropriate precautions during home visits or community outreach

  • Maintains professional standards and accountability while working autonomously

  • Demonstrates self-motivation, initiative, and problem-solving skills

Internal Candidates- A minimum of 6months relevant employment working in Crosscare homeless services.
